Chapter 13 Verification and Request for Discharge

In addition to completing a postpetition instructional course concerning personal financial management, Chapter 13 debtors must also complete and file Local Form 2083-1  Form for Verification and Request for Discharge - Local Rule 2083-1(m).  This form is to be filed within sixty (60) days of the Notice of Completed Plan filed by the Chapter 13 Trustee.
