Forms used in the Bankruptcy Court District of Utah are classified into two groups: National Forms and Local Forms.
National Forms [1] are standardized forms used in United States Bankruptcy Courts. Examples of some National Forms include: Voluntary Bankruptcy Petition, Statement of Financial Affairs and Schedules, Statement of Current Monthly Income, and all other forms required to file bankruptcy.
Local Forms [2] are specific court forms that have been customized for use in the Bankruptcy Court District of Utah. Examples of some Local Forms include: Notice and Opportunity for Hearing, Certificate of Service, and Amendment coversheets.
