Official Bankruptcy Forms will be replaced with substantially revised, reformatted and renumbered versions effective December 1, 2015.  The new forms are part of a forms modernization project that was begun by the Advisory Committee on Bankruptcy Rules in 2008.  The new forms introduce different versions of case opening forms for individual debtors and non-individual debors and  use a variety of fonts, checkboxes, and other formatting techinques to distinguish data.

The goal in this forms modernization project is to simplify the language to make it more understandable to both the legal community and debtors who file without the assistance of an attorney.  The Bankruptcy Court strongly encourages the bankruptcy community to become familiar with the new forms in order to be prepared for December 1, 2015.
